Purchase Orders
Agencies should use Form CO-94, Purchase Order and
indicate P.A. 77-405 for the
authorization code.

Please specify the billing address, delivery address,
name & phone number of the purchaser writing the order.
Quantities, sizes, colors, time periods, or other
requirements should be noted where applicable. Check the catalog for each

Payments
CCPA will
invoice agencies off the purchase order. Payments should
be made to CCPA by check. Payments terms for state purchase
orders are net 45 days unless otherwise specified.

or Services Not Listed
The
CCPA Contracts Program represents more than sixty (60)
community rehabilitation and behavioral health providers.
If the product or service your agency needs is not listed
in our catalog, please call us, as we may be able to
develop a proposal to meet your needs.
